Health Care for the Homeless 12th Street Clinic
Eligibility Guidelines
- An individual who lacks a fixed, regular, and adequate night-time residence, and
- An individual who has a primary night-time residency that is:
- A supervised publicly or privately operated shelter designed to provide temporary living accommodations; or
- An institution that provides a temporary residence for individuals intended to be institutionalized; or
- A public or private place not designed for, or ordinarily used as, a regular sleeping accommodation for human beings; or
- An individual who is doubling up with friends or relatives on a temporary situation and is not paying rent.
People who are at risk for homelessness will also be considered on a limited basis.
Payment
Our HCH Clinic provides health care regardless of ability to pay. A sliding scale based on family size and income is used to determine the charge for each patient. Patients that are at 100% or less of the national poverty guidelines are charged a small fee. No HCH-eligible person is denied services if unable to pay. We accept Kid Care Plan A (Medicaid), Kid Care Plan B (CHIP) and some private insurance plans.
