A National Idea
The nationwide Health Care for Homeless program was modeled after a 19-city demonstration program funded by the Robert Wood Johnson Foundation and the Pew Memorial Trust. The demonstration program began in 1985 and there are over 175 federally funded Health Care for the Homeless providers in the United States today.

A Community Effort
Community Action Partnership of Natrona County's Health Care for the Homeless Clinic is part of an intricate network of agencies within the community that work together to meet the needs of the area's homeless population. HCH Clients are referred to and from organizations that assist individuals and families with financial resources, insurance, education, housing, food, clothing, specialty medical care, and much more. The community works together in order to support the forward movement of persons on the path from homelessness to economic self-sufficiency.
An Individual Focus
Each homeless person that comes to our clinic has a unique history, set of needs, and capacity to resolve his/her situation. For this reason, we treat our clients as individuals, providing them with services particular to their needs through primary health care, mental health and substance abuse therapy, and outreach and case management.
