Clinic Services
Our clinic helps to remove health-related obstacles that prevent individuals and families from resolving their homelessness. Our clinic has developed a vast network of partnerships with medical, dental, and vision specialists throughout the community so that our patients' medical care is not limited to the services that we are able to provide directly through Physician Assistants, Nurse Practitioner, and/or Registered Nurses. The HCH Clinic is able to offer:
- Primary Health Care
- Preventive Health Care
- Specialty Medical Referrals (Limited)
- Medication
- Dental (Limited)
- Vision (Limited)
We not only provide traditional medical care, we also address the underlying issues: shelter and housing, income, job skills, education, family relations, substance abuse and mental illnesses. The HCH Clinic is able to offer:
- Outreach - We search out homeless people, on the street or in shelters, to help them obtain desperately needed health care.
- Case Management - We assess client needs and support services and make appropriate referrals to other community resources. Our HCH Clinic staff and our Community Action Outreach staff also work closely to coordinate services between offices.
- Mental Health and Substance Abuse Services - Our therapist has very close linkages to a number of in-patient and out-patient treatment facilities throughout the State of Wyoming.
- Health Education - Educational videos, pamphlets, brochures and training is provided to broaden awareness of health-related issues.
- Transportation - Transportation can be arranged for our patients. We provide CATC bus tickets and taxi vouchers. Our 12-passenger van service runs scheduled routes to and from several shelters and other community resources.
